- Verifiable Personal information (name & valid phone number) + 2 provider references you´ve seen in the past 6 moths (email, website and link to their ads)
- Employment information, Employer name, Job Title, Company Website
- Tell me about your self
- Preferred Date, Time, Duration of booking
- Any Special Request
Cancellation Policy: In the event, you have to cancel i appreciate 48 hours notice
24 hours o less -50% cancellation fee
Deposits can be applied to another date within 30 days of initial hooking
In the rare event that i cancel, you will be fully refunded along with several apologies & promises for a great time when we do meet
Failure take care of cancellation fee´s or ´no show´s without a proper 36 hours notice will result in being placed on an internacional blacklist & the termination of our friendship
